One of the most common reasons women wait months (or years) to get pelvic floor help is thinking they need to 'go through their OB first.' In New Jersey, you don't. Here's exactly how it works, so you can stop waiting.
New Jersey has direct access
New Jersey is a direct access state for both physical therapy and occupational therapy. That means you can book an evaluation with a licensed PT or OT — including a pelvic floor specialist — without a physician referral. You don't need permission from your OB, PCP, or urogynecologist to start.
When a referral does matter
The referral question is really an insurance question. It depends on your plan, not on the law:
- In-network insurance plans (HMOs especially) often require a referral to reimburse — check your plan.
- Out-of-network / cash-pay clinics (like Modern Movement) do not require a referral, ever.
- HSA and FSA accounts do not require a referral — you can pay with those cards directly.
- Superbills for out-of-network reimbursement usually require a diagnosis code, not a physical referral.
Why cash-pay is often the better deal for pelvic floor therapy
Even with insurance, most in-network pelvic floor clinics: (1) run 4-week wait lists, (2) double-book you into a shared gym, (3) give you 20-30 minutes with the therapist, and (4) still bill you a copay every visit. Cash-pay 1:1 care means:
- You start this week, not next month.
- One full hour, one-on-one, in a private room.
- Fewer sessions overall because the care is actually individualized.
- You can use HSA/FSA dollars — that's tax-free money designed for exactly this.
- A superbill for possible out-of-network reimbursement.
How to actually get started (5 minutes)
- Book an evaluation online — no referral needed.
- Fill out the intake form (medical history + goals).
- Show up for a private 1:1 session in Basking Ridge or in your own home.
- Leave with a plan, home program, and clarity on how many sessions you likely need.
- If you want a superbill, we generate one after each visit.
What if my OB / PCP said 'just do kegels'?
That's the most common thing we hear, and it's what keeps most women stuck. Physicians receive very little pelvic floor training in med school — it's not a knock on them, it's a scope-of-practice issue. Pelvic floor therapys specialize in exactly this. You don't need their blessing to get specialized care.
